Please fill out the form below to register for a library card. A library card will be mailed to you within 3 business days. A temporary access code will be displayed and sent to your email. Use your temporary access code to access some of our services before your card arrives in the mail.

If you have already registered with any public or community college library in San Mateo County, you do not need to complete this form. Please visit one of our locations to obtain a replacement library card. Identification and proof of current home address is required.
